In health services, treatment models of acute and chronic illnesses are diverging increasingly. Two different models of care are therefore likely to emerge in the near future. Due to increasing specialisation, acute care will become more centred on the individual (the patient-centred care model). Care of chronic ailments will be organised by city or region in a number of locally delivered therapies, which will increasingly be based on the participation of patients (the patient-driven care model). Inpatient and outpatient treatment processes, which are gaining in complexity, need coordination in terms of space and time.
